  1. Medical Certificate: A medical certificate is a written statement from a physician which is issued for a variety of reasons including certification of a patient’s illness, certification of a customer fitness, or certification of a patient’s recovery from a medical condition.
  2. Fit to fly Certificate: Some airlines require a medical letter/certificate confirming that the patient’s medical condition is currently stable and suitable to travel.
  3. Medical Records: Medical records are information concerning a patient’s health care such as recent doctor visits, discharge summaries, medications, allergies, and lab results.
  4. Insurance reimbursement papers: Such as the medical certificate, medical records, and receipts along with the detailed information.

What do I need to prepare for my medical travel in Thailand?

  1. Check if your passport is valid at least 6 months before traveling date and if you need a visa to travel to Thailand.
  2. Consider purchasing travel insurance to make your trip enjoyable and travel with peace of mind, to protect you against unseen events such as flight cancellation, delayed flights, lost baggage, emergency on board, theft etc. Make sure the insurance you purchase covers your special medical needs during travel.
  3. Financial matters: Manage how you will finance your medical payment, such as cash or credit card, and make sure you have additional cash in the local currency. Before going abroad, notify your bank and your credit card companies of your medical travel, to prevent banks from treating the transaction as suspicious and blocking your account.
  4. Ensure that you keep all important medical documents with you also medications in your hand luggage. A medical summary is very helpful to pack in case of any emergency during your travels.
  5. Check the weather forecast for Bangkok, Thailand.
  6. Check if it is necessary for you to get certain vaccinations.
  7. Once you have purchased your flight tickets, make sure you have sent the itinerary to those will pick you up from the airport.
  8. Required documents.
  9. Ensure that you have contact details of your healthcare provider, your assistance or your transport provider.
  10. Copies of your passport and immunization records is required, keep them in a separate location. In case of lost passports, it is much easier to get a replacement of your passport if you have a copy.

  2. AstraZeneca or Covishield by AstraZeneca and the University of Oxford, SK Bioscience (South Korea), Siam Bioscience, and Serum Institute of India (Covishield) – 2 doses needed / 4-12-week interval;
  3. Pfizer–BioNTech or Comirnaty by Pfizer Inc. and BioNTech 2 doses needed / 3-week interval;
  4. Janssen or Janssen/Ad26.COV2.S by Johnson & Johnson Services, Inc. – 1 dose needed;
  5. Moderna by Moderna Inc. – 2 doses needed / 4-week interval);
  6. Sinopharm or COVILO by Sinopharm Co., Ltd. – 2 doses needed / 3-4-week interval);
  7. Sputnik V by the Gamaleya Research Institute of Epidemiology and Microbiology – 2 doses needed – 3-week interval).

Required documents

  1. Copy of the patient’s passport with a certified signature (newly signed).
  2. Copy of the companion’s passport(s) with certified signature(s) (newly signed) [One patient can come with up to 3 companions].
  3. Fit-to-travel or Fit-to-Fly certificate issued by a licensed doctor in the current country, within 72 hours before departure. (DO NOT attempt to get this item before the approval is confirmed.)
  4. Certificate of COVID-19 free within 72 hours before departure. (DO NOT attempt to get this item before the approval is confirmed.)
  5. Insurance policy covering the treatment of COVID-19 incurred in Thailand, medical transportation and repatriation if needed, the minimum amount of USD 100,000 for each patient and companion.
  6. A Vaccination Certificate, stating its name, a manufacturer and lot number of vaccines with a provider’s or center’s name.
  7. Proof of financial status, e.g., bank statement.
  8. Letter of intention and consent for quarantine under AHQ. Letter of approval to be hospitalized / quarantined in the hospital for 7-10 days (Prepared by Jitta Life Wellness Center and signed by the patient and/or companions.)
  9. Certificate of Entry (COE) for Medical Tourism purposes from Thai Embassy in your country, after the approval of all documents above from the Ministry of Health in Thailand.
Thailand Plus Application; From 14 January 2021, all travelers who have received COEs are required to download and register via the Thailand Plus Application. They are required to show a QR code generated from the registration via the Application to be scanned by the Thai authority upon arrival in Thailand. What should I take with myself?

  1. Mobile phone with a charger. Purchasing of a Thai SIM card is necessary, to download and to register covid-19 daily monitoring and tracking application “COSTE” during your stay in Thailand.
  2. Personal documents such as ID card, passport and visa.
  3. Credit/debit card and some additional cash in the correct local currency.
  4. Daily medication, if any, in original packaging; any type of information regarding any chronic diseases that you may have.
  5. Medical records, including current diagnosis, films (X-ray, MRI, CT scan, ultrasound), biopsy results and your contact information.
  6. Items for entertainment purposes during your medical travel; for example, books, music players.
  7. Facemask and a basic set of toiletries such as soap, shampoo, toothbrush and toothpaste, shaving kit, deodorant, sanitary napkins.
  8. A set of clothes in case of receiving physiotherapy and rehabilitation, if required after surgery; this may include a tracksuit and a pair of sneakers.
  9. Sleep essentials such as pajamas, earplugs and your favorite small pillow or blanket.

Recommendation for your travel (by CDC)

  1. Maintain a distance of 6 feet (2 meters) between you and others as much as possible.
  2. Avoid contact with anyone who is sick.
  3. Limit contact with frequently touched surfaces, such as handrails, elevator buttons, and kiosks. If you must touch these surfaces, use hand sanitizer or wash your hands afterward.
  4. Wear a cloth face mask.
  5. Avoid touching your eyes, nose, and mouth.
  6. Cover coughs and sneezes.
  7. Clean your hands often. It’s especially important after going to the bathroom, before eating, and after coughing, sneezing or blowing your nose.
  8. Wash your hands often with soap and water for at least 20
  9. If soap and water aren’t available, use a hand sanitizer that contains at least 60% Cover all surfaces of your hands and rub your hands together until they feel dry.

Arrival

There are steps to follow when arriving at the airport

  1. At the screening point in the arrival hall (Concourse E), the staff of the Ministry of Health will wait for you and bring you to the screening processes. You need to present the Certificate of Entry with lab test result issued no more than 72 hours prior to departure at check-in.
  2. You are recommended to download the AOT Airport application (downloaded from the App store or Google play) to report the state of health
  3. You can wait for your luggages after this screening point.
  4. The airport staff will bring you to Exit 4, and the hospital staff will then drive you to the alliance hospital. You are encouraged to self-isolation process under Alternative Hospital/State Quarantine protocol (AHQ/ASQ) for 7-14 days.

Departure

There are steps to follow when finishing your trip.

  1. If you do not fly back to your home country after discharging from the clinic within 72 hours, you need to repeat the Covid-19 test at the same hospital that you stayed for quarantine (AHQ).
  2. The clinic or AHQ will prepare a “Fit to fly specifically to Covid-19” certificate if the result is negative ( Undetected).
  3. Our clinic staff will contact you via email for following up about the Covid-19 test and symptoms after you arrived at your home for 21 days.

Instructions for Returning Passengers

  1. You must quarantine yourself for 14 days once you get back from Thailand. During quarantine time, patients should strictly comply with the following guidance:
    – Monitor your body temperature and record twice a day, if there are symptoms, such as cough, fever, sore throat, difficult of breathing, please immediately seek for medical attention.
    – Do not allow visitors.
    – Wash your hands frequently with soap and water or alcohol-based hand sanitizer (if they are dirty), especially after touching secretions such as mucus, saliva and sputum while coughing or sneezing or after urinating or defecation.
    – Get plenty of rest in a room that is not too cold and is well-ventilated. Drink enough water. Avoid drinking cold water.
    – Try to eat healthy meals.
  1. After the quarantine period, it is recommended that the patient should continue wearing a facemask for at least 1 month and always maintain good personal hygiene.

Thailand Covid-19 Situation Reports

Due to the COVID-19 pandemic, Thai authorities have imposed regulations regarding a permission to enter the Kingdom of Thailand for those who need to receive medical treatment in Thailand except for treatment for COVID-19. The patient, including their accompanying escorts (no more than 3 at maximum) must comply with the regulations and mandatory quarantine period upon arrival.

Thailand AQ (Mandatory Quarantine)

Fully vaccinated travelers from non-listed countries and those travelers from listed countries who booked flights entering Thailand through Bangkok airport, are required to undergo the mandatory quarantine in Bangkok for 7 days. For vaccinated travelers from listed countries who do not wish to quarantine, you must enter using the sandbox program.

Additionally, for international travelers who are unvaccinated or not fully vaccinated, they will be required to undergo the mandatory alternative quarantine of 10-14 days in a designated quarantine hotel in Bangkok before they can travel to other parts of Thailand.

Vaccinated travelers will be able to enter Thailand without quarantine under the Phuket Sandbox Program , Phuket Sandbox 7+7 Extension Program  and Samui Plus Sandbox Program , subject to a certificate of vaccination or so-called “vaccine passport”.

Starting November 2021, there is a proposal for a Sandbox Program for Bangkok, Chiang Mai, Chonburi (Pattaya), and Hua Hin. The details and the official guidelines are yet to be announced in the coming days.

Protocols for the Quarantine in the hospital (AHQ)

  1. You (and your companion) will be subject to the quarantine process at the alliance hospital from the day of arrival into the kingdom of Thailand for at least 7-14 days at any of the accredited by the Ministry of Health.
  2. You will be allowed to leave the room for the medical investigations and medical/dental procedures throughout the quarantine period.
  3. Fever and Respiratory tract symptoms will be monitored on a daily basis. The RT-PCR test for Covid 19 will be performed 2 times (Day 0 and Day 6-9-13 before discharge)
  4. If the result is positive (Covid19 detected) at any time, the treatment can commence right after the test and you will be cared for in the hospital under your own expense. (Under the insurance coverage, conditions may apply)

  10. All exam rooms and treatment rooms are disinfected by a disinfectant sprayer machine also changing a disposable sheet on every bed and chair immediately after used for each client.
  11. Provide good ventilation and have efficient air purifiers in the pubic area and each room.
  12. Limit the number of clients to an appropriate proportion to prevent transmission.
  13. We use disinfectant products that have been approved by the Thai FDA.
  14. All doctors and staffs at Jittalife Wellness Center have Covid-19 vaccinated.
  15. Build trust by testing SARS-CoV-2 – ATK (COVID-19 Rapid Test) to all doctors and staffs every 3-7 days to secure from COVID-19.

Patient’s Rights

We can provide better medical care when you and your family work together as partners with our staff. While it is our responsibility to advise you of your rights as a patient; you also have responsibilities in your treatment and care. We urge you to ask questions, cooperate, and be proactive in the medical care plan. If you have questions or concerns, please discuss with any staff member or manager.

While you are in our clinic, you have certain rights as a patient:

  1. Patients have fundamental rights to get medical care from healthcare professionals without discrimination as stated in the constitution.
  2. Patients have the rights to receive truthful and adequate information about the illness, investigation, treatment, as well as benefits and risks from healthcare professionals in an appropriate language before making a decision to consent except in a case of emergency and life-threatening conditions.
  3. Patients with life-threatening conditions have the rights to receive immediate medical attention from healthcare professionals without the need to request for help.
  4. Patients have the rights to know name and profession of the responsible healthcare providers.
  5. Patients have the rights to ask for an opinion of another healthcare professionals and have the rights to request for a change to other healthcare professionals or medical facility that comply with the terms and conditions of respective health insurance.
  6. Patients have the rights to confidentiality of medical information except when a consent is provided or when the information is required by healthcare professionals to provide direct medical benefits.
  7. Patients have the rights to be completely informed before making a decision to participate in human subject research.
  8. Patients have the rights to make a request for information in their medical record that comply with wellness center rules and regulations as long as the request does not violate the rights of others.
  9. A parent or legal guardian may exercise the rights on behalf of the patient who is under 18 years old or physically or mentally incompetent.

Patient’s Responsibilities

  1. Patients are responsible for asking for information and potential risks before signing an informed consent for diagnostic or treatment procedure.
  2. Patients are responsible for providing truthful and complete health and other information to the responsible healthcare professionals.

  • Patients are responsible for not leaving the clinic facility without a permission from the doctor or nurse to ensure patient safety.
  1. Patients are responsible for treating healthcare professionals, other patients, and visitors with courtesy and respect.
  2. Patients are responsible for informing about health insurance benefit with proper documentation to the clinic.
  3. Patients are responsible for being aware of the following facts:
  • Healthcare professionals who perform standard and ethical practice are protected by law and have the rights to be protected from unfair accuse.
  • Medical care refers to a current medical practice that is based on scientific evidence to show higher benefits than risks to the patient.
  • Medical care cannot diagnose, prevent, or treat all diseases or condition.
  • Any medical care can result in unexpected consequences despite the fact that the healthcare professional has been extremely careful in any situation.
  • Any test for disease screening, diagnosis, and treatment monitoring might have inaccurate result because of technological limitation and other uncontrollable by current standards.
  • Healthcare professionals have the rights to choose an evidence-based medical treatment option in a given context and limitations as well as to consult or refer the patient as appropriate.
  • Healthcare professionals may consult or refer the patient who are not in an emergency or life-threatening condition.
  • Medical care provided by healthcare professionals might be compromised, if necessary, health information is not disclosed by the patient.

Any act of physical violence, verbal threats or intimidation towards the clinic staff will not be tolerated. Anyone, who uses violence against clinic staff, is prosecuted and may be banned from using clinic services.
